Community Partners

In order to better serve people requiring home care services in Central New York, we are pleased to build and foster close working relationships with many organizations throughout the immediate geographic region and beyond. Included among them are the following:

Onondaga County Step Up to Stop Falls: This coalition’s goal is to learn more about the effects of falls among the Onondaga County aged population, as well as what measures can, and should, be taken in order to help prevent falls. Core coalition members include: Onondaga County Department of Aging and Youth; Nascentia Health; Aurora of Central New York; St. Camillus Health and Rehabilitation Center; and the Salvation Army. Other key coalition members include: Loretto Health Support; Loretto Pace; St. Joseph Hospital; Crouse Hospital; Excellus; Upstate University; and Interfaith Works. Projects initiated by Onondaga County Step Up to Stop Falls include: community strength and balance exercise programs; medication education; professional practice change (working to educate professionals in falls risk screening); in-home assessments; and public information.

Care Transitions (Crouse Hospital): Using a program developed by Crouse Hospital’s Heart Failure Transition Coach, a team from Crouse Hospital and Nascentia Health work together to provide a full continuum of cost-efficient, high-quality care for cardiac patients. The team has developed a communications hand-off tool that provides a seamless transition from the hospital to home care for heart failure patients. The overall goal is to reduce hospital readmissions by presenting a detailed, clear assessment of the patient’s clinical status at the time of discharge.
